For some other providers, their first thought is probably injection or medication. Then if those don't work, they recommend surgery. For many patients, they don't even know that they have acupuncture as an option. At the end, they've exhausted everything. Then they do a web search and find acupuncture may work for their conditions. That's almost like the other way around. Like a reversed process. ST: Right. That paradigm needs to be flipped over. WH: That's right, that's right. ST: How do large institutions embrace integrative medicine? WH: I think that nationally, large institutions like Emory University Healthcare, are embracing the idea of integrative medicine now after seeing the trends patients advocate for them- selves and providers advocate for the patients. However, during discussions to set up integrative medicine programs, economics still come up. ST: So, you also see how revenue is driving treatment in large American healthcare systems? WH: In big hospitals, like Emory, our overhead is huge. In order to balance the budget, we have to be more creative in finding ways of integrating services such as acupuncture, while at the same time, making sure the finance is not hurting our bottom line. ST: I am sure that that's true. Any hospital needs to see more financial benefit. WH: Yes, yes. It is sad for healthcare, in my opinion, but that's just my personal opinion as a physician. Our primary concern should be to optimize patient outcomes, not to maximize finance. It's very unfortunate hospitals do have to get the numbers right. It's the US healthcare reimbursement policy that probably needs to be changed, to encourage more patient-centered care, more non- invasive treatments, like in Europe. ST: What about patients themselves advocating for this type of treatment along with medical doctors? What is it going to take? WH: You mean in the future? ST: Yes, to see your vision of integrative healthcare and that of many other people come to fruition. What is it going to take? Is it going to take a really strong push by patients saying, No, wait a minute. I don't want this surgical option first. What can I try before I go under the knife? WH: I do believe that the biggest push comes from the patients. Then, of course, provider education is very important. From this point of view, the most important thing is to educate both patients and providers. We will also need to make some changes to our current medical education system. Nowadays, not many medical residents going through their training know about integrative medi- cine or holistic medicine. They are still trained to prescribe this pill for this condition when they see this symptom. Instead, we need to educate them about medical practice for people, not for symptom- atology. Seeing someone with GERD doesn't mean automatically prescribing Nexium. The symptom could be controlled probably by stress reduction, diet modification, or fresh ginger tea. We need to expose our residents to various types of medical systems around the world, based on clinical and research evidence. Many other medical systems exist with strong support that work as well as conventional Western medicine in certain areas of health- care. But, I'll try to avoid other terms that can potentially confuse the audience, such as San Jiao. If a journal wants to write something in very traditional acupuncture terminology, at least they should have the closest biomedical explanation after that.
