BACKGROUND Colorectal cancer screening can decrease both the incidence of and mortality from colorectal cancer. Unfortunately, participation rates remain suboptimal. We assessed whether an evidence-based lecture or a written clinical prompt at the time of a patient visit would independently increase colorectal cancer screening rates in an Internal Medicine resident clinic. METHODS Three-phase prospective cohort trial of 750 patients. The first phase assessed the baseline-screening rate. The second phase assessed the screening rate after an evidenced-based lecture. The third phase assessed the screening rate after addition of written prompts to patient charts. All 50-80-yr-old patients who met criteria for colorectal cancer screening were included in the study. The first intervention was a 1-hr evidence-based lecture addressing colorectal cancer screening. The second intervention was placing a written prompt on all eligible patients' charts reminding physicians to assess if their patient had been screened. Demographic characteristics were assessed for each cohort of 250 patients. The percentage of patients with documented intention to be screened was assessed for each cohort. RESULTS The evidence-based lecture did not significantly improve overall attempted screening rates or colon imaging rates relative to baseline. The clinical prompt significantly improved attempts at screening relative to baseline (39.6-67.6%) (P < 0.0001). Ordering of colon imaging rates also significantly improved after instituting the prompt from 24% to 46% (P < 0.0001). CONCLUSIONS Clinical prompts are superior to evidence-based lectures for improving physician colorectal cancer screening practices. These prompts are simple low-cost measures that can improve quality of care. Although during my first visit, I had to wait a really long time, I feel like I was overall really satisfied with my visit after I had left. I had been having really bad rib pains for a couple days now (probably 4 now?). I had hurt my rib after I leaned over the middle console in my car and I think that probably bruised my ribs. Dr. Lu was very gentle and examined my ribs very carefully. He told me I most likely bruised my rib bone and tore my cartilage. For treatment, he had me do some electrical therapy and gave me some muscle relaxants. I then came in again the next day and had the same treatment again. First of all didn't know that seeing a PA who is totally in a pissed off mood and completely uninterested would total an over $300 exam fee billed to my insurance Completely brushed off and had to ask that I have blood work done. Here is the best part. I have a phlebotomist who maybe it was her first day but wow she was bad. Super painful and poked me ten times before getting it. Bruised all over. Then put it in the wrong tubes. So I of course had to call about my blood work and they said it was messed up and neeeded to do again. I said ok I will stop in tomorrow after work since going out of town the next day (still in a lot of pain by the way). Come in...same girl...poked all over including my hand which then looked like it was slammed in a car door for a week. She tried multiple times to blame me for being dehydrated when i drank 3L bottles of water to stay hydrated at work (and my kidneys function fine). I told her that but she kept arguing. I hope the room was cleaned well because my blood was on my other arm, my pants and the floor. I dont have anything contagious but that is ridiculous. Then I call again after not hearing anything because why would you call the patient? They messed up AGAIN and I have no answers. I call back fuming that I need to talk to a manager. Twice I was ignored and told I would receive a call back but never did. also got the $100 bill I owed after insurance. They should have paid me $100 for being such idiots.
