Services advertised range from second medical opinions to full team treatment, including a clinical psychologist's attention to a patient's mood and emotional states, relaxation techniques such as massage and meditation, acupuncture, sleep, therapy and gentle physical activity. In the case of cancer and addiction treatment centers, these facilities even offer inpatient organic cuisine and yoga by the seaside. While patients nationwide are demanding to be treated as more than walking packages of organs, blood, bones, and nerves, just how much of this national trend in medical marketing is true? And what does the hungry-for-comprehensive-care patient really get to choose from the smorgasbord of treatments? A Roundtable Discussion Mainstream Medicine and Integrative Patient Care: Who Gets to Choose? To answer these questions, we decided to take a look at refractory conditions, such as chronic pain and insomnia, that often require complementary treatment such as physical therapy, acupuncture, chiropractic, and massage, in addition to the treatment of second- ary conditions including depression, poor or inadequate nutrition, and lack of correct sleep hygiene, to name but a few. Patients experiencing these conditions may benefit the most from an integrative treatment environment. For the purpose of this discussion, we chose to focus on met- ropolitan Atlanta, the 9th largest metropolitan area in the United States with a population of 5,268,860 as of the last census taken in 2010 (1). We looked at Emory University, the city's largest health- care institution, which is comprised of 6 hospitals, 200 clinics, and 1800 physicians. Emory's hospitals are consistently ranked among the best nationwide, with 10 adult specialties ranked in the nation's top 3% (2). Its healthcare system is vast and continues to expand so we thought we could find some answers to our questions here, among the biggest and best national institutions. Two Emory physicians, both of whom are also assistant professors in the School of Medicine, entertained our topic thoughtfully and with candor. A third clinical voice, from outside of this vast system, comes from an Atlanta physician who is, in his own words, an advocate of universal healthcare, and who specializes in providing care for patients with refractory conditions. We noted that Atlanta is located at the nexus of the Bible Belt and its residents, though often touted as progressive because of the city's ethnic and cultural diversity, remain somewhat conventional in attitude in comparison to similar populations within large northeastern or western cities. Change, we posited, may just come about a bit more slowly in a city and a region where conservative attitudes prevail.
